提交正式投诉
A formal complaint is a document filed by a Complainant or signed by the 第九条协调员, alleging sexual 骚扰 against a Respondent 和 requesting that the 机构合规办公室 investigate the allegation(s) of sexual 骚扰. 提交正式投诉没有截止日期. 在正式投诉提交的时候, 投诉人必须参与, 或者试图参与, LMU或JFWA的教育项目或活动. 一旦正式投诉被提交, the University is required to investigate the allegations contained in the formal complaint.

This form may be used to request an appeal for a dismissal of a formal complaint under LMU's Sexual Harassment/第九条 Policy 和 Grievance Process or to request an appeal for a hearing panel's written determination of responsibility under LMU's Sexual Harassment/第九条 Policy 和 Grievance Process.
